Susan G. Komen San Diego’s Breast Health Diagnostic Services Program

Patients

If you meet the program eligibility requirements for the Komen San Diego Health Diagnostic Services Program, please complete the following:

1. Visit your regular medical provider for a clinical breast exam (this is an exam where your provider physically checks your breasts by feeling them for anything abnormal). If you do not have a regular medical provider or medical home, please click here to find one near you. 

2. Once you have seen your provider, they will determine which diagnostic test you need (if any) and will write a referral for that test. If you are unable to pay for that test, your provider can fill out an Authorization Request Form and submit it to Komen San Diego for approval.

3. Once the authorization form is approved it will be sent with your doctor’s referral to the imaging center where you will receive the diagnostic test. After the test is done, the imaging center will send the bill to Komen San Diego.

Note: If you have already seen your medical provider and they have already given you a referral for a breast diagnostic test, but you are unable to afford  the test and you meet the eligibility requirements below, you can contact your referring medical provider and ask them to submit an Authorization Request Form for the Komen San Diego Breast Health Diagnostic Services Program or contact Lizzie Wittig at (858) 573-2760 x104 or lizzie@sdkomen.org.

 

Patient Eligibility

To receive services from the Komen San Diego Breast Health Diagnostic Program, the patient must:

  • Be a resident of San Diego County
  • Not be eligible for Every Woman Counts (OR Every Woman Counts patients who need an MRI)
  • Not be eligible for Medi-Cal
  • Be at or below 300% of the federal poverty guidelines, see the chart below for income guidelines.
